JOINT AFFIDAVIT

            We, _____________ and ____________, both Filipinos, of legal ages, and residents of ______________, Philippines, having been duly sworn in accordance with law, hereby depose and state:

1.       That we know the person ___________ as the same has been a close acquaintance for a long period of time;

2.       That we know for a fact that ____________ and ____________ were lawfully joined together on _____________at ____________ in a marriage ceremony solemnized by _______________;

3.       That we are likewise aware that the date of marriage appearing in the Certificate of Live Birth issued for their child, ____________, is not the true and correct one as they have not yet taken each other as husband and wife at the time of his birth;

4.       That it was discovered but recently when ____________ requested for a copy of their Marriage Contract from the Civil Registrar that the said document has not yet been registered probably through oversight and so the said Marriage Contract was only registered last ____________;

5.       As such, we execute this affidavit to certify or attest to the truth of the foregoing circumstances and for whatever legal purposes that this affidavit may serve.
